Develop an incentivized technical solution aimed at empowering and improving the care of patients living in the community with Chronic Obstructive Pulmonary Disease. The solution will aim to empower and improve the care of patients living with COPD in the community. The solution will provide patients with a tailored domiciliary activity programme that incorporates all of the key components of a standard Pulmonary Rehabilitation programme.
