by Wellola
Wellola's Portasana is a modularised, interoperable patient communication platform, designed to compliment existing systems in a given healthcare ecosystem. Portasana® offers 11 digital communication modules, designed to reduce pressure on the healthcare system and streamlines patient care by equipping individuals with virtual care pathways, fully curated by the organisations that care for them. It enhances communication between healthcare providers and patients, ensuring a well-informed treatment journey, while also improving operational efficiencies and cutting costs. The platform supports patient education and self-management via tools such as education, online assessment (PROMS/ PREMs/ PIFU etc.), messaging and symptom tracking among others. This innovation shifts care from hospitals to communities, personalizing treatment pathways with its flexible, interoperable system.
